Documents required for processing Death Claim Cases in Post Offices

Death claims Without nominee certificates are required.

• Account Holder's Death certificate
• Copy of Aadhaar Card 
• Copy of Post Office Passbook on first and last page
• SB3 card (Hard copy or Finacle copy whichever is available)
• Latest Finacle ledger
• Legal heirship Certificate
• Copy of Aadhaar of all the family members which is available in legal heirship with sign and phone number
• Copy of Claimant's Aadhaar
• Copy of claimant POSB Account first page
• Two witnesses (should be the same station as the claimant/account holder) - Copy of Aadhaar with sign and phone number
• Two sureties Aadhaar, Payslip with sign and phone number

• Form 11 in two copies (Witnesses sign required)
• Form 13 Affidavit, stamp value 20/-(Legal heirs sign required)
• Form 14 Disclaimer, stamp value 20/- (Legal heirs sign required)
• Form 15 Indemnity, ** stamp value 100/- (Sureties and witnesses sign required)
